We are no longer working with health insurance, however we will be happy to provide you with a detailed receipt, also known as a "superbill", with all the information and codes you need for submitting to your insurance company for reimbursement.
We are accpeting PIP (car accident insuance) on a limited basis. Please call for more information.
There is a space on the online intake when you schedule your appointment to let us know you'd like an insurance receipt. These are sent via email, usually on the weekend following your appointment.
